Job Summary: The Community Mental Health Center (CMHC) Community Support Specialist will provide care coordination, ensure comprehensive recovery support linkages to legal, medical, social services, housing, education/employment, assist clients receiving SED/SMI/SUD/COD mental health services attain their highest level of independent functioning. Responsibilities include, but are not limited to, supporting, developing and/or applying the treatment/service plan and facilitating compliance. Serves as a member of appropriate interdisciplinary treatment team. Reports to CMHC Program Coordinator. This is a grant funded position.
Qualifications: Bachelor’s degree in Psychology, Social Work or a related field required; previous experience in a mental health setting preferred. A Provisional Certification through the Department of Mental Health (PCCSS) must be acquired and documentation presented to Human Resources within 90 days from hire date. Completion of full certification must be obtained with two (2) years from provisional issue date. Employee must demonstrate ability to recognize consumers’ individual needs based on medical conditions, age (infants, pediatrics, adolescents, young adults, middle-aged and geriatric), limitations and planned procedures. Good communication skills are necessary. Must have valid driver’s license in Mississippi and proof of auto insurance. This position has been determined to have an occasional or very low risk of occupational exposure to blood-borne pathogens.
