Job Summary: The Patient Care Coordinator I serves as an integral member of the interdisciplinary team for the Certified Community Behavioral Health Center in Hinds County. The Patient Care Coordinator I’s primary responsibility is to respond to the needs of adult, children/adolescent consumers using a patient-centered approach that removes silos of care and supports an integrated system. This position will provide whole-person care coordination across medical, behavioral, and social service community systems. The Patient Care Coordinator I reports to the Patient Care Coordinator II.

Qualifications: Bachelor’s Degree (Master’s Degree preferred) in Mental Health, Behavioral Science or health-related field preferred. Minimum of four (4) years in Mental Health, Behavioral Science or other health related field, with some experience in recognizing consumer’s individual needs and limitations. Must possess a mental health license or certification, or be eligible for such. Must possess strong interpersonal and communication skills and ability to work harmoniously with staff, community agencies and leaders; commitment to the community mental health center program concept; innovative and flexible approach to develop new programs.
