Job Summary: Works in collaboration with a physician and mental health therapist to assess the needs of the individual in crisis and to implement a plan of care. Functions as a Case Manager for an assigned caseload of adults with serious and persistent mental disorders. Responsibilities include, but are not limited to, responding to crisis calls in the community, supporting, developing and/or applying the treatment/service plan and facilitating compliance. Serves as a member of appropriate interdisciplinary treatment team.
Qualifications: Bachelor’s degree in Psychology, Social Work or a related field required; previous experience in a mental health setting preferred. A Provisional Certification through the Department of Mental Health (PCCSS) must be acquired and documentation presented to Human Resources within 90 days from hire date and (CSSS) must be obtained within two (2) years from the date provisional certification is obtained. Employee must demonstrate ability to recognize consumers’ individual needs based on medical conditions, age (infants, pediatrics, adolescents, and young adults, middle-aged and geriatric), limitations and planned procedures. Good communication skills are necessary. Must have valid driver’s license in Mississippi and proof of auto insurance. This position has been determined to have an occasional or very low risk of occupational exposure to blood-borne pathogens.
