Job Summary:

Functions as a case manager by conducting street outreach visiting local shelters, soup kitchens, and other areas where homeless migrate. Establish relationships with inpatient facilities and the Crisis Stabilization Unit. Must be SOAR (SSI/SSDI Outreach, Access, and Recovery) trained and obtain documented certification. Create and maintain accurate spreadsheets to track client contact, services provided, referrals, and referral sources.  Reports to Director/Coordinator of Adult Services

Qualifications: Bachelor’s degree in Psychology, Social Work or a related field required; previous experience in a mental health setting preferred. A Provisional Certification through the Department of Mental Health (PCCSS) must be acquired and documentation presented to Human Resources within 90 days from hire date and (CSSS) must be obtained within two (2) years from the date provisional certification is obtained. Employee must demonstrate ability to recognize consumers’ individual needs based on medical conditions, age (infants, pediatrics, adolescents, and young adults, middle-aged and geriatric), limitations and planned procedures. Good communication skills are necessary. Must have valid driver’s license in Mississippi and proof of auto insurance. This position has been determined to have an occasional or very low risk of occupational exposure to blood-borne pathogens.
